Job summary

Curana Health seeks a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care in skilled nursing and senior living facilities. You will provide direct patient care, promote wellness, manage chronic conditions, and coordinate with other providers.

This full-time role requires travel among assigned facilities. You will build treatment plans, conduct rounds, educate residents and families, document in EMR, and participate in quality initiatives to reduce

Qualifications

• Licensed NP or PA with current board certification in their field
• Master’s degree required from accredited program
• Strong communication and leadership skills
• Ability to work in a team and travel between facilities

Responsibilities

• Perform comprehensive patient assessments and develop treatment plans.
• Conduct routine rounds and monitor residents’ health status.
• Coordinate care with facility staff and other providers.
• Educate residents, families, and staff on wellness and disease management.
• Maintain EMR documentation and ensure regulatory compliance.
• Participate in quality improvement initiatives to reduce readmissions.
