Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to manage a panel of long‑term care members in a nursing home setting. You will deliver collaborative primary care, develop individualized care plans, and participate in facility‑level care team meetings with families and staff.

You will coordinate with PCPs, manage prior authorizations, and support quality initiatives while maintaining high‑quality, member‑centered care across transitions.

Qualifications

• 6+ months experience as a Nurse Practitioner or Physician Assistant.
• MSN as a Nurse Practitioner from an accredited program OR Master's in nursing/related field.
• Experience in nursing home settings is strongly desired.
• Proficient with computer/technology systems.

Responsibilities

• Provides collaborative primary care to a panel of members in long‑term care.
• Develops and implements care plans with an emphasis on ISNP Model of Care.
• Manages prior authorizations, Medicare services, and quality gap closure.
• Acts as a trusted advisor to members, families, and facility staff.
• Communicates care plans and changes with PCPs and interdisciplinary team.
• Maintains accountability and autonomy in the nursing home environment.
