Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to manage a panel of long-term care residents. You will conduct comprehensive assessments, craft individualized care plans, and engage in care-team meetings while maintaining clear communication with families and staff.

Qualifications

• 6+ months experience as a Nurse Practitioner or Physician Assistant.
• MS in Nursing as a Nurse Practitioner from a nationally accredited program OR Master’s Degree for a Physician Assistant.
• Experience in the nursing home environment strongly desired.
• Computer/Technology proficiency.
• Ability to engage with physicians, members, and family members.
• Self-directed but works well in a team.
• Excellent clinical skills—assessment, diagnosis, treatment, prescribing for chronically ill.

Responsibilities

• Provides collaborative primary care to a panel of members.
• Delivers ISNP Model of Care activities and advanced care planning.
• Manages and coordinates health plan clinical operations and member communication.
• Serves as a trusted advisor to members, families, and staff.
• Maintains communication with members’ PCPs and care teams.
• Supports market goals and balances member caseloads.
• Establishes relationships and functions autonomously in nursing home settings.
• Delivers high-quality, member-centric care.
