Job summary

Gather Health is expanding its primary care model with NP/PA-led clinics serving older adults in Boston communities. You will manage a patient panel through clinic, telehealth, and in-home visits, supported by a collaborative team including Guides and EMT/Paramedics for post-discharge and home safety checks.

You’ll work in a value-based care setting with a base salary plus performance incentives, focused on building long-term relationships and addressing social determinants of health.

Qualifications

• Licensed NP or PA with primary care experience.
• Active Massachusetts medical license, or ability to obtain one prior to start.
• Clinical proficiency in evidence-based primary care.
• Ability to connect authentically with patients from diverse backgrounds.
• Comfort with EHR and care technology platforms.
• Strong ethics and discretion in complex, sensitive situations.
• Collaborative spirit — we work without hierarchy and welcome all perspectives.

Responsibilities

• Clinic and telehealth visits with your panel (no double-booking, no 10-minute slots).
• Care team huddles with Guides and Medical Technician Partners.
• Remote administrative time for documentation, care coordination, and team communication.
• Home-based care coordination supported by your MTP team (you direct; they deploy).
