Job summary

Curana Health seeks a licensed Nurse Practitioner or Physician Assistant to lead proactive primary care for seniors in skilled nursing and related communities. You’ll assess, diagnose, and manage health needs, building meaningful patient relationships while collaborating with physicians and care teams.

In this role, you’ll practice with autonomy, use advanced technology, and travel between facilities. A valid NP/PA license and board certification are required; Curana supports CME and career

Qualifications

• Licensed as a Nurse Practitioner or Physician Assistant in the relevant state.
• Board certification in your field.
• Leadership, interpersonal and problem-solving skills expected.

Responsibilities

• Deliver primary care with comprehensive health assessments and individualized treatment plans.
• Diagnose and manage acute and chronic conditions focusing on prevention and wellness.
• Develop strong patient relationships through regular rounds and team collaboration.
• Document accurately in EMR and ensure regulatory compliance.
