You will provide high-quality clinical care to members in nursing homes, senior housing, and assisted living settings, delivering comprehensive assessments, care planning, and patient education.

The role requires active NP/PA certification and state licensure, with DEA registration and reliable transportation.

Qualificações

• Active NP/PA healthcare certifications as applicable.
• Current unrestricted state licensure and DEA registration as required.
• Access to reliable transportation and valid driver’s license.

Responsabilidades

• Conduct comprehensive assessments including medical history, vitals, and exam.
• Support short-term sub-acute rehabilitation patients.
• Ensure accurate ICD-10 documentation.
• Collaborate with interdisciplinary care team.
• Lead advanced illness and end-of-life discussions.
• Educate patients and caregivers.
