Position Details:
  • Remote role. Candidate must reside in Florida, Miami-Dade or surrounding areas. Must have four years or more of Pediatric experience and LPN licensure.
Responsibilities:
  • Assess, plan, implement, and coordinate care management activities based on member needs to provide quality, cost-effective healthcare outcomes.
  • Develop or contribute to personalized care plans and educate members and families on services and benefits.
  • Coordinate with healthcare providers and community resources to ensure timely access to services.
  • Monitor progress and collaborate with healthcare teams for care plan revisions.
  • Provide psychosocial support and resource linkage to members and caregivers.
  • Identify barriers to care and implement interventions for members with higher care needs.
  • Review referrals and assessments to develop appropriate care plans.
  • Maintain all member information and activities for compliance.
  • May perform on-site member assessments and provide education to care teams and members.
  • Serve as SME or preceptor for new staff as needed.
  • Other duties as assigned.
Qualifications:
  • Bachelor’s degree with 4–6 years of related experience, or equivalent.
  • Graduate from an accredited nursing school if holding clinical licensure.
  • Current state clinical license preferred.
  • For CMS Florida licensed practical nurses: minimum four years pediatric experience; or Master’s in social work with at least one year pediatric care experience.
Pay Range:

$26.50 - $47.59 per hour
