Job summary

Los Angeles Jewish Health is seeking a Clinical Pharmacist to support the Brandman Centers for Senior Care (BCSC) program in California. The role emphasizes medication management, external pharmacy review, PDE reporting to CMS, and collaboration with PBM to optimize benefits.

The candidate will hold a PharmD with a CA license and 2–3 years of managed care pharmacy experience, including senior care exposure. This is a full-time position offering comprehensive benefits and professional growth.

Qualifications

• PharmD degree required.
• Current California pharmacist license required; TB screening and health exam.
• 2–3 years of managed care clinical pharmacy or related experience; senior/elder care experience preferred.

Responsibilities

• Understand External Pharmacy Review requirements per QAPI plan.
• Review participants’ drug regimens and profiles.
• Collaborate with Medical Director to develop review processes and educate prescribers.
• Report findings to QAPI Committee quarterly.
• Ensure appropriate medication management and utilization through chart reviews.
• Coordinate with PBM and contracted pharmacy for benefits and PDE reporting.
• Identify cost-effective pricing for brand and generic meds.
• Provide enrollment/disenrollment data to pharmacy and facilities.
• Lead P&T meetings and conduct monthly audits of stock.
