Job summary

Quality Moments Behavioral Health seeks a Senior Care Case Manager (LSW, LPC preferred) to provide compassionate care and companionship in skilled nursing facilities. The role may be filled part-time or full-time, focusing on activities that enhance social, emotional, and behavioral well-being of aging adults.

The ideal candidate values dignity, personal connection, and reliability, and has experience with Alzheimer’s patients.

Qualifications

Must be at least 21 years old.
Diploma or GED required.
Desire to improve quality of life for aging adults.
Must have experience working with Alzheimer patients.
High level of commitment, dependability, and interpersonal skills.
Prior experience as STNA, CNA, or in a nursing/long-term care setting is preferred.
Prior experience in social work, counseling, human services, or a related field is preferred.
Valid driver’s license and reliable transportation with current automobile insurance meeting state minimum liability requirements.
Previous EHR system use, and professional documentation experience are helpful.

Responsibilities

Provide therapeutic activities and companionship to individuals in skilled nursing facilities.
Develop activities addressing social, emotional, and behavioral well-being.
Build meaningful relationships and support aging adults in daily living.
