Collaborating with nursing staff, families, and care teams, you will document care in the EHR and ensure reliable transportation when required, upholding dignity and respect for aging adults.

Qualifications

• Must be at least 21 years old.
• Diploma or GED required.
• Experience with Alzheimer patients preferred.
• Strong dependability and interpersonal skills.
• Experience in social work, counseling, or human services preferred.
• Driver's license and reliable transportation with insurance.
• Experience with EHR systems and documentation helpful.
