Job summary

Vitra Health in Massachusetts is seeking two Registered Nurses who are passionate about community engagement and helping clients access the care they deserve. As an RN, you will conduct intake, assessments, and visits, educate clients and caregivers, and support health maintenance and navigation of services in clients' homes.

You will travel within the assigned region, collaborate with the care team, document thoroughly, and promote compassionate, dignified home-based care.

Qualifications

  • Massachusetts RN license required.
  • Two years of direct care experience with elders or adults with disabilities preferred.
  • Valid driver's license and reliable transportation.
  • Fluency in English; second language preferred.
  • Strong communication, documentation and organizational skills.

Responsibilities

  • Complete intake, annual assessments and admission visits.
  • Document client assessments and multidisciplinary plans of care; communicate with PCP and providers.
  • Educate clients and caregivers; assist in health management navigation.
  • Conduct in-home visits; provide caregiver education and skills training.
  • Review and update plans of care; reconcile medications as needed.
  • Collaborate with the multidisciplinary team to address client needs.
  • Maintain a culture of customer service and quality care.

As a Registered Nurse, in addition to executing assessment and admission visits, you will provide non-clinical support, educating clients and caregivers. Key Responsibilities
Essential Functions
  • Complete intake and annual assessment and admission visits.
  • Complete all required documentation including client assessment forms and multidisciplinary plan of care and communicate professionally with client PCP and other healthcare providers when necessary.
  • Review all information in the client's admission binder, ensuring understanding as well as educating the caregiver on use of the MH required caregiver communication tool or electronic visit verification system depending on program participation.
  • Complete regular, in-home visits, caregiver education/skills training and assess health and environmental changes
  • Conduct semi-annual visits, reviewing and updating existing plan of care and corresponding goals, medication review and reconciliation and review caregiver communication tools.
  • Address concerns regarding environment, psychosocial status and changes in physical health.
  • Participate in multidisciplinary team meetings and work collaboratively to address client needs.
  • Perform other duties within the scope of practice as assigned by management.
  • Foster culture of customer service and commitment to quality care.
  • Serve as a brand ambassador for Vitra reflecting our vision, mission, and values.
  • Show a genuine interest and compassion for the communities we serve.

What We Are Looking For
  • Willingness to travel within assigned region required.
  • Valid driver's license and reliable transportation required.
  • Licensed as an RN by Massachusetts Board of Registration in Nursing, required.
  • Minimum of two-year recent experience in the direct care of elders or adults with disabilities.
  • Fluency in both English and second language strongly preferred.
  • Case management, behavioral health, social services and/or community health worker experience preferred.
  • Strong communication, documentation and organizational skills required.
  • Must be solution oriented and able to multitask.
Work Environment And Physical Requirements
  • Office-based work in a clean well-lit environment with fluctuating temperatures near others.
  • Client-facing field staff and community liaisons require frequent travel to client homes or community settings; ability to drive safely; work in client homes may involve varying temperatures, odors, allergens, pets, and other environmental factors.
  • Requires substantial periods of repetitive work utilizing a computer, monitor, keyboard, and mouse.
  • Requires lifting and carrying equipment and supplies weighing up to 35 pounds; requires pushing and pulling equipment and supplies weighing up to 35 pounds; requires walking and standing; requires frequent sitting more than 75% of the workday; requires the ability to negotiate stairs; requires visual acuity and manual dexterity to operate equipment.
