Job summary

The role requires an LCSW license and an MSW from a CSWE-accredited program, plus 3–5+ years in hospice or home health and 1–2+ years of leadership.

Qualifications

• Maine state licensure as LCSW.
• MSW from CSWE-accredited program.
• 3–5+ years in hospice, home health, or palliative care.
• 1–2+ years of leadership or supervisory experience.
• Strong knowledge of hospice philosophy, Medicare/Medicaid regulations, and psychosocial care.
• Excellent communication, leadership, and conflict resolution skills.
• Ability to manage multiple priorities in a fast-paced, patient-centered environment.

Responsibilities

• Provide clinical supervision and mentorship to behavioral health and social work staff.
• Support psychosocial assessments, treatment planning, crisis intervention, and bereavement services.
• Review documentation to ensure best practices and ethical standards.
• Serve as a resource for complex cases, including family dynamics and end-of-life care planning.
• Perform staff evaluations and assist with performance management and disciplinary actions.
• Lead interdisciplinary team (IDT) meetings to integrate care across services.
