Job summary

Harboroaks seeks a clinician to manage patient cases with a high degree of clinical knowledge and expertise. You will assure that patients meet care criteria from admission through discharge and initiate authorization with network providers, while maintaining knowledge of clinical information through case staffing and care conferences.

Qualifications

• Bachelor's degree in Social Work, Counseling or related human service field required.
• Licensed health care professional with mental health experience (MSW; LCSW; RN; CNS; ARNP; Registered Mental Health Professional (MHP); Counselor (MA; MFT)) required.
• Three or more years' experience working in a behavioral health setting required.

Responsibilities

• Assure that patients meet care criteria from admission through discharge.
• Initiate authorization with network providers.
• Maintain knowledge of clinical information through review of documentation and care conferences.
• Perform concurrent review with network providers and monitor medical record documentation.
• Liaise with physician and treatment team to document all aspects of treatment.
• Lead information flow between levels of care to ensure smooth discharge.
• Coordinate resources with community services and agencies for continued care decisions.
• Perform other functions and tasks as assigned.
