Qualifications

• CA license required (LCSW, LMFT, LPCC, RN, or PsyD).
• RN requires an associate's degree or BSN.
• Master's degree required for non RN-licensure.
• Minimum five years of relevant experience, including three in behavioral health.
• Certified Case Manager preferred.
• Experience conducting psychosocial assessments and developing care plans.

Responsibilities

• Help members navigate care systems and link to behavioral health providers.
• Assess emotional well-being and coordinate resources and interventions.
• Provide crisis intervention and counseling for complex cases.
• Consult physicians and care managers for patients with psychosocial needs.
• Educate, advocate, and connect with community resources.
