Job summary

The Commonwealth of Pennsylvania is recruiting for an Aging Care Manager 3 in Monroe County to help safeguard and support older adults. The role emphasizes collaboration, service coordination, and ensuring timely interventions to improve safety, health, and independence.

Key duties include reviewing needs, arranging services, developing care plans, and coordinating resources. The position offers full-time hours, potential telework after probation, and compliance with protective service laws.

Qualifications

• One year as an Aging Care Manager 2 or Aging Program Assessor.
• One and one-half years of professional human services, law enforcement, or nursing experience, plus a bachelor’s degree in a related field.
• An equivalent mix of experience and training that includes six months in related human services, law enforcement, or nursing roles.

Responsibilities

• Review categories of need and begin investigations within required time frames per Older Adults Protective Services Act.
• Reduce or mitigate risk to older adults and complete investigations within regulated timelines.
• Develop care plans, complete assessments, and document contacts using the SAMS program.
• Arrange medical, psychological, and social assessments and connect individuals with appropriate services.
• Provide education on protective services and mandatory reporting through public presentations.
• Meet with supervisors, attend network meetings, and assist with related task force activities.

Bachelor's degree in social sciences / behavioral sciences / human services or related field
