The role requires nursing program graduate, experience in post-acute admissions, and strong clinical assessment and writing skills. The position supports a team environment with interdepartmental collaboration and periodic weekend coverage.

Qualifications

• Excellent writing and clinical assessment skills.
• Good working relationships with staff and referral sources.

• Bilingual (English/Spanish) a plus.
• Thorough knowledge of medical terminology relevant to geriatric populations.
• Ability to perform comprehensive bio-psychosocial assessments.
• Strong interpersonal and verbal communication skills.
• Organized and able to work independently on projects.
• Detail oriented with follow-through.
• Proficient with Word and Excel.

Responsibilities

• Reviews PRIs and Screens and coordinates all components of the admissions process with the interdisciplinary team.
• Consults with hospital staff to review potential applicants for admission.
• Performs pre-admission assessments and gathers medical and social information.
• Reviews census reports for discharges, admissions, and readmissions and follows up.
• Documents all applicant clinical and psycho-social assessments.
• Verifies payer sources (Medicare, Medicaid, managed care, insurance).
• Interacts with HMO Case Managers prior to admissions to review needs.
• Obtains authorizations for managed care organizations as needed.
• Provides weekend and holiday admission coverage as needed.
• Navigates multiple referral systems (internal and external).
• Assists potential residents and families by coordinating tours and answering questions.
• Maintains accurate documentation per State and Federal regulations.
• Assists other departments with projects in a team environment.
• Performs all other duties as requested.
