Job summary

United Community Action Partnership, Inc. is seeking a Self-Sufficiency Case Manager in Marshall, MN. The role focuses on empowering individuals and families to achieve personal goals through comprehensive case management and housing stabilization support.

You will assess needs, create action plans, document contacts, and advocate for participants while collaborating with local service providers to promote self-sufficiency.

Qualifications

• Minimum 2–4 years of education and/or experience in a related field.
• A four-year post-secondary degree in Human Services preferred.

Responsibilities

• Provide case management services to help individuals and families set and achieve goals.
• Conduct assessments and develop individualized plans, assist in housing stabilization.
• Keep detailed records and maintain participant confidentiality.
• Support participants in becoming independent from public assistance.

WHAT YOU'LL DO

• Empower & Support:Provide case management services that help individuals and families set and achieve personal goals.
• Guide Participants:Conduct assessments, develop individualized plans, and assist in stabilizing housing situations.
• Manage Cases:Keep detailed records, document all contacts, and maintain participant confidentiality.
• Promote Self-Sufficiency:Support participants in becoming independent from public assistance.
• Community Engagement:Build relationships with service providers, attend networking meetings, and advocate for participants.
• Stay Organized:Ensure accurate documentation, track referrals, and complete necessary reports on time.

EDUCATION AND EXPERIENCE

• Minimum of two (2) - four (4) years of education and/or experience in a related field required.
