Job description

We are looking for motivated, energetic individuals to join our team as Licensed Practical Nurses! As an employee with us, you will be part of a large, dedicated staff devoted to caring for residents in a County-owned facility. We are seeking applicants able to succeed in a fast-paced environment and multi-task!

Self-Scheduling employees will be required to work a minimum of 72 hours per month, with a minimum of 24 worked on off-shifts (3p.m. -11 p.m. and 11 p.m.-7a.m) and 24 hours being worked on weekends (7p Friday – 7am Monday) per month. Hours not to exceed 1,000 in a 12 month period or 32 in one week.

Self-Scheduling employees will be given a schedule during orientation. After orientation, employees must submit a monthly schedule of availability to the Staffing Coordinator for approval. This position provides the opportunity for successful candidates to have a flexible schedule and competitive pay rate!

First Shift: $37.12 per-hour
Second, Third & Weekends: $42.17 per-hour

No vacation, personal, or sick time will be accrued. No retirement plan offered. Self-Scheduling employees will not be eligible for shift differential, meal allowance, holiday pay, or health insurance. Other guidelines, including but not limited to attendance rules, to apply and are subject to change; guidelines must be acknowledged at hire and adhered to or program eligibility will cease.

ESSENTIAL FUNCTIONS OF JOB:

The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.

  1. Ensures that policies and procedures are complied with by nursing personnel assigned.
  2. Participates in the admission, discharge and transfers of residents as required.
  3. Observes for changes in residents’ condition, notifying the RN/ RN supervisor for further assessment and notifies the physician and resident’s family or representative and documents accordingly.
  4. Obtains verbal orders and transcribes physician orders to medical record and carries out orders as written. Takes off written orders when completed by the physician.
  5. Prepares and administers medications as per physicians’ orders and observes for adverse effects.
  6. Maintains narcotic record and count.
  7. Provides a detailed shift to shift report to support continuity of care.
  8. Performs wound treatments as per physicians’ orders, observes for changes, documents accordingly and notifies parties as applicable with changes.
  9. Schedules follow up appointments for residents and transportation needs as indicated.
  10. Provides direct care skills such as ADL care, feeding, colostomy changes, tube feedings, wound care, suctioning, IV administration, etc. in accordance with current policies and procedures.
  11. Performs rounds to ensure resident needs are being met and personnel are performing their assigned duties.
  12. Collaborates with other members of the interdisciplinary team as needed to create plan of care and ensure residents’ needs are holistically met.
  13. Informs appropriate departments of concerns or complaints and files grievances as necessary.
  14. Reports any incidents or unusual occurrences to the supervisor, Assistant Director of Nursing or Director of Nursing and participates in the investigative process as needed.
  15. Documents condition of hospital returns, new admissions, changes in condition, incidents, infection, appointment follow up, GDR’s and transfer of residents.
  16. Completes user defined assessments as assigned in the EHR.
  17. Ensures all ordered testing is completed and follows up as indicated.
  18. Participates in end of life care as required.
  19. Ensures that there is adequate stock of medications, supplies, equipment and notifies appropriate personnel of needs.
  20. Initiates, reviews and updates care plans as required.
  21. Participates in the survey process when inspection surveys arise.
  22. Treats all residents with dignity and respect. Promotes and protects all residents’ rights.
  23. Establishes a culture of compliance by adhering to all facility policies and procedures.
  24. Complies with standards of business conduct, as well as state/federal regulations and guidelines.
  25. Follows appropriate safety and hygiene measures at all times to protect residents and oneself.
  26. Maintains confidentiality of protected health information, including verbal, written, and electronic communications.
  27. Participates in all life safety and emergency drills and trainings. Fulfills responsibilities as assigned during implementation or activation of the facility’s emergency plans.
  28. Reports work-related injuries and illnesses immediately to supervisor.
  29. Follows established infection control policies and procedures.
  30. As a condition of employment, completes all assigned trainings and skills competencies.
  31. Maintains work area(s) in a clean and properly arranged manner at all times.
  32. Attends/completes annual and monthly in-services and trainings as required.
OTHER DUTIES OF JOB:
  1. Assumes other duties as deemed necessary by the Nursing Supervisor – including but not limited to assuming the role as “lead nurse” in accordance with the facility guidelines and LPN scope of practice in PA.
  2. May be pulled to perform as a certified nursing assistant dependent upon staffing needs.
  3. Promotes teamwork, mutual respect, and effective communication. Promotes positivity and active daily problem solving.
  4. Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  5. Must not pose a threat to the health and safety of other individuals in the work place.
  6. Ability to work beyond normal working hours and on weekends and holidays when necessary.
  7. Attends meetings, trainings, seminars as required.
  8. Performs other tasks as necessary in the absence of other co-workers.
  9. Reports noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel. Reports any retaliation or discrimination to HR or compliance officer.
  10. Reports any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to appropriate persons. Protects residents from abuse, and cooperates with all investigations.
  11. Reports any occupational exposures to infectious materials and/or hazardous chemicals in accordance with facility policy.
QUALIFICATIONS:

Minimum Training and Experience Required to Perform Essential Job Functions:

  1. High school graduate or equivalent required.
  2. Must be a graduate of an approved LPN/LVN program.
  3. Must hold a current unrestricted license as a Licensed Practical Nurse (LPN) or GPN in the Commonwealth of Pennsylvania, or a compact license with the ability to practice in PA.
  4. Must hold a current CPR certification.
  5. Prior experience in long-term care preferred but not required.
SUPERVISION RECEIVED:

From the Registered Nurse, Nursing Supervisor(s), Assistant Director(s) of Nursing, Director of Nursing, and others.

SUPERVISION GIVEN:

To unit clerks, unit assistants and certified nursing assistants on the nursing unit to ensure standards of care in accordance with license.

WORKING CONDITIONS:
  1. Must be able to work effectively in a healthcare setting and nursing unit environment.
  2. Works indoors, with adequate lighting, fluctuating temperatures and ventilation.
  3. Subject to frequent interruptions.
  4. Works within average exposures to noises, stress, dust and dirt.
  5. Potential exposure to blood borne pathogens or communicable diseases.
  6. Subject to exposure to infectious waste, diseases and/or conditions which may include Influenza, Coronavirus, Hepatitis B, Scabies (not an exhaustive list)
  7. May be subject to hostile or emotional residents, family members, visitors or personnel.
  8. May work beyond assigned working hours, including weekends and holidays, when necessary.
PHYSICAL/MENTAL CONDITIONS:
  1. Must be able to move throughout the work day.
  2. Must be able to push pull and maneuver medical carts.
  3. Must be able to lift/transfer residents in accordance with Care Plans.
  4. Ability to do heavy lifting.
  5. Ability to use a variety of nursing and medical equipment per facility policy and manufacturer’s recommendations.
  6. Must be willing to comply with all facility, CMS, CDC and PA DOH policies and regulations regarding infection control and prevention.
  7. Must possess leadership and mentoring abilities and willingness to work harmoniously with other personnel.
  8. Communicates with medical and nursing staff, and other departments.
  9. May be pulled to perform as a certified nursing assistant dependent upon staffing needs.
  10. Meets general health requirements according to facility policy, including medical and physical exams and checking immunity status to various infectious diseases.
  11. Ability to bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed in the position. Must be able to move intermittently through facility throughout the workday. Ability to walk, stand, and sit as-needed throughout workday.
  12. Must be able to pay close attention to detail and concentrate in a moderately stressful environment.
  13. Ability to understand and actively encourage the Westmoreland Manor Code of Conduct and the Westmoreland County Code of Ethics.
  14. Must possess the ability to record, convey and present information to the general public, explain ordinances and procedures and follow instruction.
  15. Dexterity requirements range from fine manipulation of hands/fingers and coordinated movement of feet/legs/torso to carry out job duties.
  16. Ability to perform basic arithmetic.
  17. Ability to read, write, speak and understand the English language.
  18. Must be a supportive team member, contribute to and be an example of team work.
  19. Ability to assist in evacuation of residents during emergency situations.
  20. Must be willing to undergo COVID testing as directed by CMS/CDC and/or PA DOH.
  21. Ability to properly wear surgical mask, N95 and/or eye protection or PPE required for the duration of shift as directed by CMS/CDC and/or PA DOH.
  22. Strong listening skills and ability to deal with conflict with professionalism and courtesy.
  23. Basic computer skills, including ability to navigate electronic medical record systems, electronic learning platforms, and phone systems.
  24. Positive interpersonal relationship skills, including with persons of all ages and cultures.

Mathematical Requirements:

  1. Ability to add, subtract, multiply, divide, and calculate decimals and percents.

Language Ability & Interpersonal Communication:

  1. Ability to record and deliver information, and follow instructions.
  2. Ability to comprehend and interpret a variety of documents including medication administration record, narcotic flow sheets, lab reports, treatment administration record, Plan of Care, policy manual, diagnostic and drug manuals, patient charts, lab books, etc.
  3. Ability to complete a variety of documents including, quality assurance audits, pharmacy sheets, maintenance requisitions, lab slips, residents medical records, supply orders, pharmacy reorder sheets, etc. using prescribed format and conforming to all rules of punctuation, grammar, diction, and style.
  4. Ability to use and interpret medical terminology.
  5. Participates in QAPI or facility assessment activities as needed, such as carrying out duties assigned as part of a performance improvement committee.
  6. Ability to communicate effectively with other staff, residents, and the general public verbally and in writing.
  7. Must be able to speak and understand the English language and to follow oral and written instruction.
  8. Must possess the ability to function independently, have flexibility and the ability to work effectively with the general public and co-workers.
  9. Must possess effective interpersonal and communication skills.
  10. Must possess the technical knowledge to operate a computer at an intermediate skill level and use other office equipment to perform job duties.
  11. Must possess the ability to make independent decisions when circumstances warrant such action.
  12. Must possess the ability to follow instruction and complete required forms and documentation without error.
