Search360 is seeking a Self-Direction Assistant/QIDP to support individuals and families in Lincoln Square, Chicago. The role involves providing SDA onsite at Search locations, in homes, and in the community, with occasional travel in northern Cook County.

Responsibilities include case management, scheduling visits, and billing, reporting to the Enrollments and SDA Manager. Reliable transportation and schedule flexibility are required for this full-time position.

Formación

• Strong written and verbal communication and presentation skills.
• Familiarity with Microsoft Office programs.
• Knowledge of DHS Rules 120, 119, 116, 115 and CQL standards.

Responsabilidades

• Provide case management and service facilitation for SDA funding.
• Schedule required visits in a timely manner.
• Submit billing for services in a timely and accurate manner.
• Manage service agreements for SDA caseload and outside agencies.
• Act as liaison between fiscal agent, employer and personal support workers.
• Conduct outreach and maintain professional contacts.
• Maintain caseload files in a timely, organized manner.
• Demonstrate leadership and teamwork within Search.
