Job description

Primary Functions

Provide assistance and instruction in activities necessary to promote and maintain independence for individuals with intellectual/developmental disabilities who choose to Self-Direct their services. The Self Directed Program provides as-needed care to individuals as assigned, to meet their daily needs and enable them to function at the highest possible level.

Specific Duties

  • Assist individuals with specific facilitative outcomes as described in each individualized Self-Directed Plan. These duties may include personal care, community integration, money management, advocacy, employment needs, and respite.
  • Seek opportunities to meet individuals' psychosocial needs through care planned and spontaneous actions.
  • Provide reliable, safe, high-quality services as directed, in a thorough and caring manner.
  • Implement a person-centered approach to services.
  • Follow universal precautions, proper infection control, sanitation, and safety standards in all activities.
  • Complete paperwork and web-based timesheets promptly in accordance with payroll processing.
  • Communicate with supervisor and Circle of Support about individuals' habilitation plans that may need revisions or updates.
  • Communicate effectively with individuals, families, and professionals involved in the individuals' care.
  • Work collaboratively with team members to meet the needs and desires of the individual served.
  • Respect individuals' dignity, privacy, property, religion, and culture.
  • Maintain accurate records of contacts with individuals.
  • Provide interventions in crisis situations and report any allegations of abuse or neglect immediately.
  • Attend mandatory training sessions.
  • Provide transportation as needed, following safety procedures.
  • Maintain flexibility in schedule based on individual needs.
  • Be dependable, arriving on time and prepared for work.
  • Perform other duties as assigned by the individual or supervisor, if driving is required.

Qualifications

  • High School Diploma or equivalent.
  • One year of practical experience working with individuals with intellectual/developmental disabilities preferred. A combination of experience and education may be substituted at the discretion of the Executive Director.
