Job summary

Responsibilities include implementing the Staff Action Plan, documenting service times in eVero, attending training, and advocating for the person.

Qualifications

• High school diploma or GED.
• Must meet Approved Driver Status.

Responsibilities

• Health and safety of individuals supported.
• Provide individualized services including respite, teaching, and skill development in home and community.
• Collaborate with the person’s team and advocate for the person per the Staff Action Plan.
• Follow objectives in the Staff Action Plan discussed with the person and team.
• Attend training to improve services; complete requirements within timeframes.
• Document service times and supports in Racker’s eVero system; record mileage as required.
• Use smartphone for documentation and tasks as needed.
• Input goals and service input for the individual.
• Communicate with the person and their team; maintain professionalism in behavior and dress.
• Assume related duties as assigned.
