Job summary

Springbrook is seeking an SDS Program Coordinator to assist the Assistant Director in administering Self-Directed (SD) Fiscal Intermediary services, maintaining budgets, billing, and compliance across Medicaid and corporate standards.

You will collaborate with COS teams, coordinate service plans, supervise documentation, and support staff while ensuring person-first practices. This role requires an NYS driver's license, ability to lift 50 lbs, and flexible scheduling.

Qualifications

• Bachelor Degree’s required (Human Services preferred, others will be considered).
• Preferred 1 year’s experience with individuals with developmental disabilities or related field.
• Must be able to work a flexible schedule.
• Duties require professional verbal and written communication skills.
• Proficiency in or knowledge of using Microsoft Excel, Outlook and Word; ability to learn other software.
• Must be able to lift 50 lbs. independently and assist with heavier lifting.
• Meet requirements to drive Springbrook vehicles; reliable transportation and travel may be required.
• New York State Driver’s License is required.

Responsibilities

• Adhere to person-first philosophy and Springbrook policies at all times.
• Meet with individuals to coordinate service plans within FI level of support.
• Audit SD plans/budgets to ensure documentation is complete.
• Maintain billing and payroll documents per DOL and Medicaid requirements.
• Assist with New Employee Orientation and training for SD staff.
• Attend meetings and travel to Springbrook locations as needed.
• Track grievances and report concerns to supervisor.
