Supplemental Health Care in Forest Park, IL is seeking a local contract School Physical Therapist to support a partnering school district. You will assess students, develop and implement educational and therapy programs, and work within a multidisciplinary team to determine eligibility and monitor progress.

The position runs 34 weeks with two hours per week on two-hour days, and may involve travel between campuses or supervising assistants.

Qualifications

• Illinois Physical Therapist License required.
• American Heart Association BLS required.
• Prior experience with pediatrics or in a school setting is preferred but not required.

Responsibilities

• Develop and implement educational and therapy programs based on evaluation results.
• Evaluates students to identify needs and determine eligibility for therapy services.
• Collect therapy data, maintain accurate student records, and monitor progress toward student goals.
• Work collaboratively and communicate with school interdisciplinary team and student guardians.
• May be asked to travel between school locations; may supervise assistants.
