Job summary

Nursing Responsibilities

• Collaborate with school nurse/director of nursing and the parents to develop and implement individual health care plan for student assigned.
• Maintain an accurate health record on student
• Document health related activities and treatments on daily logs and/or computer based health record program as appropriate.
• Document administration of medications, treatments, first aid and illness care provided to student
• Communicate with parents as necessary regarding the health status of student
• Maintain medical equipment and supplies as appropriate in the classroom per student Individualized healthcare plan (IHCР)
• Collaborate/cooperate with community providers and state agencies to meet the health and social service needs of student.
• Be a resource to the student, parent, principal and teachers in matters pertaining to health.
• Identify health and safety hazards in the classroom and advise school Nurse, Director of school nursing and teacher(s).
• Other duties as assigned by School Nurse, Nurse leader per students IHCP.
• Provide personal care, assistance and supervision, as needed, for 1:1 students including: dressing and undressing, toileting, including diaper change, grooming, feeding, positioning, transfers: i.e. may include heavy lifting, hygiene
