Job summary

Homecare with Heart is seeking an attentive Skilled Nurse to perform patient care in home settings under physician orders and the plan of care. Shifts run from 6:30a-4p, with part-time or full-time options and a supportive team environment.

Qualifications

  • Licensed nurse in the State of Ohio, in good standing.
  • Current BLS CPR Certification.
  • One year or more experience as an RN/LPN in home health preferred.
  • Must be able to read and understand various documents used by the agency including plan of care, flow charts, patient charts, agency policies and procedures.
  • Knowledge of Medical Terminology is required.
  • Must be able to use a computer, laptop or tablet.
  • Requires math skills including ability to add, subtract, multiply and divide whole numbers.
  • Employees must always practice patient confidentiality to protect the patient’s privacy.

Responsibilities

  • Administer medication in accordance with standards of practice.
  • Encourage social/rehabilitative skills and encourage self-care and independence.
  • Perform patient treatments in accordance with the plan of care.
  • Assist the patient in learning appropriate self-care techniques.
  • Prepare clinical and progress notes to document care provided
  • Monitor, document accordingly and report any change in patient condition to the Clinical Director.
  • Promote activities to achieve a safe and healthy environment.
  • Observe aseptic technique when performing patient procedures/care.
  • Practice Standard Precautions at all times.
  • Work with other members of the clinical team to develop and improve the plan of care.
  • Assist the patient in achieving established goals in the plan of care developed by the RN CM.
  • Maintain productivity according to the standards of the agency.
  • Participate in patient care meetings, in-services, orientation and staff meeting.
  • Understands when to request supervisory assistance.
  • Adhere to the policies and procedures of the agency.
  • Provide service in accordance with agency’s philosophy, standards, policies and scope of practice
  • Participates in Performance Improvement and adheres to HIPAA guidelines.
  • Other duties as assigned.
