Job summary

Integrated Services for Behavioral Health is seeking a Community Behavioral Health Worker for a school-based position with Huntington Local Schools in Ross County, OH. The CBHW provides coaching, skill-building, and practical supports in home, school, and community settings, guiding access to resources and promoting recovery and resilience.

Requires a high school diploma or GED; a bachelor’s degree or 3+ years of relevant experience is preferred.

Qualifications

• High school diploma or GED required.
• Bachelor’s degree in related field or minimum 3 years direct relevant experience required.
• Experience with multi-need individuals and families is required.
• Experience partnering with local behavioral health systems, housing, legal systems, and social services is preferred.
• 3+ years of experience in related field is preferred.

Responsibilities

• Engage individuals and families in the home and community to identify strengths, barriers, and goals related to behavioral health and daily functioning
• Provide skill building, coaching, and practical supports in home, school, and community settings
• Support clients in developing routines, coping skills, emotional regulation, executive functioning skills, and healthy communication patterns
• Assist clients in navigating community resources through modeling, coaching and teaching
• Deliver care in home and community-based settings
• Collaborate with internal programs and external partners to ensure quality of care
• Maintain consistent communication with clients, caregivers, and team members to support engagement and follow through
• Uses databases and Electronic Health Record systems for accurate client documentation
• Adhere to ISBH EHR documentation policy
• Meet ISBH standards for productivity and quality of service delivery
• Maintain compliance with HIPAA, mandated reporting requirements, and professional ethical guidelines
• Participate in team meetings, supervision, and required training to support ongoing development
• Use trauma-informed, strengths-based approaches in all client and team interactions
• Travel within the designated service areas
• Perform other duties as assigned
