Job summary

Sixteenth Street Community Health Centers in Glendale, WI, is seeking a Registered Nurse for our Augustine Prep North School Clinic to provide patient-centered nursing care in an outpatient, school-based setting.

The RN will monitor student health, maintain vaccine and health records, perform triage for referrals, administer medications under provider orders, and educate students, families, and staff on chronic conditions and prevention. Strong autonomy and cultural sensitivity are essential.

Qualifications

• Registered Nurse with current license and two years nursing experience; current basic life support certification (CPR)
• Supervisory experience or demonstrated supervisory aptitude
• Strong computer skills preferred
• Ability to work autonomously
• Ability to relate well to people from diverse ethnic and cultural backgrounds

Responsibilities

• Provides patient-centered nursing care in an outpatient school-based clinic, according to established scope of practice guidelines and standards of care.
• Monitors and supports overall student health, including tracking health maintenance needs, vaccine records, and school health records.
• Provides triage assessment to walk-in patients, their families, school staff and others for referrals to Primary Care Provider.
• Develops health care plans for students with chronic illnesses and disabilities as needed.
• Administers medications under Provider orders, including immunizations, antibiotics, pain medications, topical agents, insulin, and prescribed medications for chronic conditions.
• Assesses and educates student/patient, parent and staff on chronic conditions and preventive care, integrating health promotion.
• Provides navigation support for referrals or test requests.
• Performs nursing tasks (suturing, wound care, catheterization, audiograms, phlebotomy, spirometry, etc.) with demonstrated competency.
• Provides nursing expertise and acts as a resource to Medical Assistants; may guide other staff and support patient care flow.
• Assists with telephone triage and other RN pool responsibilities as needed.
• Maintains patient privacy and medical records, ensuring confidentiality and proper documentation.
