Job summary

Sixteenth Street Community Health Centers is seeking a Registered Nurse for Augustine Prep North School Clinic in Glendale, WI. You will provide patient-centered nursing care in a school-based outpatient setting, manage student health, vaccines, and records, and perform triage for walk-in patients with referrals to PCP.

You will administer medications under provider orders, develop care plans for chronic illnesses, educate students and families, and support staff with nursing tasks.

Qualifications

• Registered Nurse with current license and two years nursing experience.
• Current basic life support certification (CPR).
• Supervisory experience or demonstrated supervisory aptitude.
• Strong computer skills preferred.
• Ability to work autonomously.
• Ability to relate well to people from diverse ethnic and cultural backgrounds.

Responsibilities

• Provides patient-centered nursing care in an outpatient school-based clinic.
• Monitors and supports student health, vaccines, and health records.
• Provides triage assessment to walk-in patients and refers to Primary Care Provider.
• Develops health care plans for students with chronic illnesses and disabilities.
• Administers medications under Provider orders (immunizations, antibiotics, etc.).
• Educates students, parents, and staff on chronic conditions and self-care.
• Provides navigation for referrals or test requests.
• Performs nursing tasks (wound care, phlebotomy, spirometry, etc.) with competency.
• Acts as a resource to Medical Assistants and supports patient care flow.
• Assists with telephone triage and RN pool responsibilities.
• Maintains patient privacy and accurate documentation.
