Job summary

Northeast Kingdom Human Services in Newport, VT is seeking a Therapeutic Case Manager to support students across the Cornerstone School day treatment program, traveling between Miller's Run, Newark, Sutton, and Burke Schools. You will collaborate with a clinical team to develop treatment and transition plans, delivering individual and family interventions in a small-school setting.

The role offers a Monday–Friday schedule, a competitive hourly rate, full-time hours, and comprehensive benefits

Qualifications

• Master's degree in human service field preferred; will consider Bachelor's with relevant experience.
• Experience with emotionally/behaviorally challenged children and youth is highly desirable.

Responsibilities

• Have or develop knowledge of local, state, and regional community resources.
• Meet with or have telephone contact with selected children and parents for evaluation, supportive interventions, crisis management and communication of significant information.
• Responsible for the coordination of clinical treatment team meetings.
• Responsible for the ongoing development and implementation of transition plans with referring agencies and/or school systems, and potential post program placements.
