Job summary

Wellness Together in Rancho Cucamonga is seeking a Certified Wellness Coach (CWC) to join our school-based mental health team. You will provide low-intensity wellness education, skill-building, and proactive support for students not in active crisis, acting as an extension of the school's mental health specialist.

Responsibilities include direct student support, case management and referrals, and collaboration with families and school staff.

Qualifications

• Associate’s Degree or higher in Social Work, Psychology, Counseling or related field.
• Active certification to be registered as California Certified Wellness Coach.
• 0-2+ years of experience in school, health, or community settings.
• Experience with students, families, and community health agencies.
• Fluency in English; second language preferred.

Responsibilities

• Direct Student Support: provide social-emotional and behavioral support through brief wellness check-ins, coaching, group work, and de-escalation.
• Case Management & Referrals: manage student caseload, provide interventions, referrals, and document interactions clearly.
• Family & Staff Collaboration: coordinate with school staff, engage families, provide community resources, attend SSTs/504s, and offer bilingual support as needed.

About the Role

Are you passionate aboutpreventative wellnessand social-emotional support for students? Wellness Together is seeking a dedicatedCertified Wellness Coach (CWC)to be an integral part of a tiered mental health support system. As an extension of the school's mental health specialist, you will provide direct support, focusing on wellness education, skill-building, and proactive support for students not in active crisis. This role is key to ensuring students receive timely and appropriate guidance to enhance their overall school success.

Key Responsibilities

As a Certified Wellness Coach, you will focus on low-intensity interventions (Tier 1 and Tier 2 support) across several vital areas:

Direct Student Support:

• Deliversocial-emotional and behavioral support servicesthrough brief wellness check-ins, individual coaching, facilitate small groups, and immediate de-escalation for students experiencing emotional distress.

Case Management & Referrals:

• Manage astudent caseload, providing necessary interventions, makingreferrals to other mental health supportswhen appropriate, andmaintaining clear, accurate documentationof all interactions.

Family & Staff Collaboration:

• Collaborateclosely withschool stafftocoordinate comprehensive student care,engage familiesand provide them withcommunity resources, attend relevant student meetings (e.g., SSTs, 504s), and offerbilingual support/translationas needed.

What You'll Need (Qualifications)

• Education:Associate’s Degree or higher, with preferred degree major in Social Work, Psychology, Counseling or a related field, and adequate work and/or volunteer experience providing services in relevant fields to meet certification requirements.
• Certification:Must have an active certification/ Meet qualifications to be registered as aCalifornia Certified Wellness Coach.
• Experience:
  • 0-2+ years of experienceworking in a school, health, or community setting, preferably with children and adolescents ages 5-18.
  • Experience working with students, families, and community health/social service agencies.
• Skills:
  • Strong understanding of adolescent development and behavioral health issues.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to establish positive, trusting rapport and maintain appropriate boundaries with students.
  • Ability to work collaboratively in a multidisciplinary team environment.
  • Proficient in English (required);Fluent in a second language (preferred).
