Job summary

Point Quest Group in Martinsville, IN is seeking a School-Based Physical Therapist (PT) for the 2026-2027 school year. This part-time, hybrid role involves delivering individualized PT services to K-12 students under IEPs and medical plans, with onsite evaluations as needed.

You’ll collaborate with the interdisciplinary team, supervise PTAs, conduct initial and ongoing assessments, and complete annual reviews.

Qualifications

• License to practice Physical Therapy in Indiana or eligibility to obtain licensure.
• Excellent written and verbal communication in English.
• DOJ/FBI Live Scan background check, as applicable by state.

Responsibilities

• Create and deliver individualized PT programs for K-12 students based on IEPs/medical plans.
• Collaborate with interdisciplinary team to set goals and accommodations.
• Supervise PTAs as needed and oversee therapy delivery.
• Conduct comprehensive evaluations of student needs and perform ongoing assessments.
• Complete annual and triennial reviews in a timely, compliant manner.
• Maintain thorough documentation and provide progress reports.
