Job summary

Ladgov Corporation seeks a licensed Physical Therapist to deliver onsite school-based therapy at Turtle Mountain Middle School in Belcourt, North Dakota. You will evaluate students, develop therapy plans, and document progress while collaborating with staff and families.

Responsibilities include screenings, progress reporting, and MDT participation; full compliance with ND health and education requirements is essential. Must be able to work onsite and meet licensure and immunization requirements.

Qualifications

• Current, valid ND Physical Therapist license.
• US citizenship required.
• Ability to provide services onsite.
• Knowledge of special education requirements and ND reporting timelines.
• Graduate of a CAPTE-accredited Physical Therapy program.

Responsibilities

• Provide direct onsite physical therapy services to students.
• Conduct screenings, evaluations, re-evaluations, and assessments.
• Develop and implement individualized therapy plans with measurable goals.
• Maintain a direct service log for each student and visit.
• Collaborate with Special Education Coordinator to establish weekly service schedule.
• Consult with teachers, paraprofessionals, staff and parents regarding student needs and progress.
• Complete required evaluations and written reports within timelines.
• Attend and participate in multidisciplinary team meetings.
• Provide recommendations regarding student equipment needs.
• Prepare quarterly written progress reports.
• Provide training and support to school staff and parents.
• Maintain confidential student information and professional conduct.
