Job summary

ADAPT Community Network is seeking a Full-time Pediatric Social Worker to support students at Forest Hills West School in New York. You will provide emotional and behavioral support, advocate for families, and help navigate educational and clinical systems to secure services.

The role requires a master's degree in social work and NYS LMSW licensure, with bilingual Spanish extension preferred. You will work with an interdisciplinary team, document cases, and participate in IEP planning while

Qualifications

• Master's degree in social work from an accredited program.
• New York State LMSW licensure required or in progress.
• Experience in school-based social work preferred.

Responsibilities

• Develops case notes, social histories, and recommendations for students.
• Assists families in accessing housing, Medicaid, and community services.
• Integrates social work interventions into school-based plans and meetings.
• Participates in interdisciplinary team meetings and IEP reviews.
• Provides emotional and behavioral support to students with disabilities.
• Communicates with families regarding attendance, medical status, and resources.
• Supervises social work interns and supports staff trainings.
• Maintains confidentiality of student and family information.
