Job summary

Ilocatum is seeking a School-Based Physical Therapist (Pediatric) in Elmira, NY. The role focuses on evaluating students, developing individualized treatment plans, and delivering pediatric physical therapy to support mobility, participation, and independence in the school environment.

You will collaborate with an interdisciplinary team, report to the Clinical Coordinator of Rehab Services, and provide coverage as needed.

Qualifications

• Licensed (or eligible) NYS Physical Therapist.
• BLS certification within 90 days of hire.
• Hospital or acute care experience preferred.

Responsibilities

• Evaluate students, create individualized treatment plans, and deliver interventions based on pediatric physical therapy best practices.
• Collaborate with interdisciplinary team and report to Clinical Coordinator of Rehab Services.
• Review referrals and assess students’ physical needs; develop and implement treatment plans.
• Teach use of mobility aids, braces, prosthetics, and adaptive equipment.
• Monitor progress and adjust plans; complete documentation (evaluations, progress notes, discharge summaries).
• Provide direction to rehab support staff and ensure safety/infection control.
