Job summary

Elmira City School District in Elmira, NY is seeking a School-Based Physical Therapist to support pediatric students' physical development in the school setting. You’ll evaluate students, create treatment plans, deliver interventions, and work with an interdisciplinary team to enhance participation in daily activities.

Qualifications

• Licensed (or eligible) as a Physical Therapist in New York State.
• BLS certification required within 90 days of hire.
• Hospital or acute care experience preferred.

Responsibilities

• Evaluate students, create individualized treatment plans, and deliver therapeutic interventions based on Physical Therapy best practices.
• Partner closely with an interdisciplinary team and report to the Clinical Coordinator of Rehab Services.
• Review therapy referrals and assess students’ physical needs.
• Develop and implement treatment plans including therapeutic exercise, gait training, soft tissue techniques, modalities, and functional training.
• Teach proper use of mobility aids, braces, prosthetics, and adaptive equipment.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with physicians and care team members to ensure coordinated care.
• Complete documentation (evaluations, progress notes, discharge summaries).
