Job summary

Family Health Care is seeking a School‑Based Mental Health Therapist to join the Grant High School Health Center in Grant, Michigan. You will deliver behavioral health services to youth ages 5–21 within a school setting, coordinating with medical providers and school staff to ensure access and continuity of care.

The role includes screenings, diagnoses, treatment planning, and evidence‑based interventions, with collaboration across disciplines to support students and families.

Qualifications

• Master's degree in Social Work, Counseling, or Marriage and Family Therapy.
• Michigan licensure as LMSW/LLMSW, LPC/LLPC, or LMFT.
• Excellent communication, organizational, and time management skills.

Responsibilities

• Provide behavioral health services to children and adolescents ages 5–21 in a school-based center.
• Conduct screenings, assessments, diagnoses, treatment planning, and evidence-based interventions.
• Deliver individual and group therapies and coordinate care with school personnel and medical staff.
• Support prevention, referral, and follow-up care; maintain EMR documentation.
• Collaborate with families and community partners to promote positive health outcomes.
• Participate in health promotion and outreach activities within the school and community.
