School Based Health Care Coordinator

Summary:

The School Health Care Coordinator is responsible for coordinating and managing health services for students within the school system. This role involves identifying, assessing, and addressing health-related barriers to student success and well-being. The care coordinator works collaboratively with students, families, school staff, primary care providers, and school-based health providers to ensure that students receive the necessary health care services and bridge gaps to healthcare.

Responsibilities:

1. Monitor and track student health progress, ensuring proper follow-up and support.
2. Provide health education and counseling to students and families to manage medical conditions and promote healthy behaviors.
3. Work closely with school nurses and health providers to support students holistically.
4. Collaborate with healthcare providers, mental health professionals, and community organizations to facilitate access to services.
5. Coordinate referrals to medical, mental health, and social services for students and families.
6. Serve as a liaison between the school, families, and healthcare providers to ensure appropriate care.
7. Advocate for students and families to access necessary resources and services.
8. Assist families in navigating healthcare systems, including insurance and appointments.
9. Provide crisis intervention during health emergencies.
10. Maintain accurate, confidential student health records and document services and outcomes.
11. Prepare reports on health trends and issues for school stakeholders.
12. Ensure compliance with privacy regulations such as HIPAA and FERPA.
13. Perform additional duties as assigned.

Other Information:

Technical Expertise:

• Experience in pediatrics.
• Proficiency in MS Office Suite (Outlook, Excel, Word); Epic or similar EMR software preferred.

Education and Experience:

• Bachelor’s degree in nursing (BSN) preferred; associate’s degree required with a plan to complete a BSN within 5 years of hire.
• Current RN license in Ohio required; pediatric or school nursing certifications preferred.
• At least 2 years of pediatric healthcare experience required; school-based or community health experience preferred.
• Basic Life Support (BLS) CPR certification required.
