Community Care Network in Rutland, Vermont is seeking a School Based Clinician to provide diagnosis, evaluation, psychotherapy and coordination for children requiring mental health supports, embedded directly in the school setting to improve social and emotional functioning.

The role involves assessments, individual/group/family therapy, crisis interventions as needed, and collaboration with teachers, families and community partners.

Qualifications

• Master's degree in psychology, counseling or social work required.
• Vermont licensure preferred.
• 2-3 years' experience providing mental health services for school-aged children.
• Proficient with Word and Outlook; EMR experience preferred.
• Knowledge of community resources and service coordination.
• Crisis management, organization and teamwork skills.
• Excellent listening, communication and writing abilities.
• Maintains confidentiality and ethical practice.
• Valid driver's license and reliable transportation.
• Criminal history background check successful.

Responsibilities

• Provide clinical assessments and interventions for students and families.
• Maintain case load and provide individual, group and family therapy as needed.
• Follow-up reintegration plans for students returning to school after hospitalization.
• Promote community understanding of mental health needs and coordinate resources.
• Make home visits to support families and facilitate agency communications.
• Coordinate services with teachers, providers, and school staff.
• Assist families in locating community resources and support services.
• Provide consultation to teachers and school personnel as needed.
• Complete documentation (progress notes, assessments, referrals) by deadlines.
• Submit unusual incident reports and follow-ups when required.
• Ensure clear communication of client care information to supervisor.
• Coordinate follow-up visits in the community and arrange transportation.
