Job Objective
This position is responsible for managing an assigned caseload of adolescent and adult patients with long-term mental illness and/or chemical dependency-related diagnoses. The case manager assesses needs, develops, implements, reviews treatment plans, and coordinates services for individuals and families. The role involves providing services and supporting mental health professionals in delivering care to individuals challenged by mental illness and/or addiction.
Responsibilities and Essential Duties
- Accepts newly assigned cases with completed diagnostic intake by a licensed provider. Conducts case management assessments.
- Develops appropriate treatment and service plans with patients to facilitate effective care and build rapport.
- Provides ongoing support and case management based on treatment plans to help patients achieve goals.
- Conducts face-to-face reviews with patients and/or staff, adjusting treatment plans as needed.
- Documents all contacts promptly, maintains case records per standards.
- Assists patients in scheduling external appointments and utilizing community services.
- Refers patients to community resources, maintains relationships with providers, and ensures timely referrals.
- Tracks referrals and documents accordingly.
- Communicates with behavioral health staff regarding patient progress and issues.
- Follows up with patients and families about referrals and testing, providing updates and instructions.
- Ensures electronic health records are current with all relevant information.
- Meets regularly with supervisors for professional development and administrative review.
- Coordinates with psychiatrists and support staff on medication management and patient status changes.
- Participates in interdisciplinary team meetings and other assigned projects.

Qualifications, Skills & Experience
- Knowledge of mental illness, addiction, assessment tools, and treatment planning.
- Excellent interpersonal, organizational, and communication skills.
- Ability to work independently and as part of a team.
- Experience with electronic health records and working with underserved populations.
Education & Experience
- Relevant work experience or education in behavioral health.
- Bachelor’s degree in social work, psychology, or related field preferred.
- Minimum two years in healthcare, preferably behavioral health, with experience with adolescent and chemically dependent populations.
