Qualifications

• 3 years of experience in social work or equivalent.
• Experience coordinating services for older adults or people with disabilities preferred.
• Knowledge of area resources for older adults and people with disabilities is preferred.

Responsibilities

• Identify needs of individuals and develop Community Health Living Plans.
• Connect participants to community resources and services.
• Coordinate opportunities for social connections.
