All County Home Health and Hospice seeks a compassionate Physical Therapist for a 13-week home health contract in San Antonio, TX. The PT will evaluate patients, plan and implement therapy, and monitor progress, coordinating with PTAs and an interdisciplinary team.

Responsibilities include home health visits, educating patients and caregivers, maintaining documentation, and ensuring compliance with standards. Requires Texas licensure and ability to travel within the service area.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current Texas Physical Therapist license.
• Home health experience preferred.
• Strong clinical assessment, treatment planning, and rehabilitation skills.
• Knowledge of home safety and fall prevention.

Responsabilités

• Conduct comprehensive PT evaluations and assess mobility, strength, balance, and functional abilities.
• Develop individualized treatment plans based on diagnoses and goals.
• Provide PT interventions to improve mobility, strength, endurance, balance, and independence.
• Deliver home health PT services per physician orders and guidelines.
• Monitor progress and modify plans as needed.
• Educate patients and caregivers on exercises and home safety.
• Document evaluations, sessions, progress, and discharge summaries in the EMR.
• Oversee PTAs and collaborate with the care team.
• Travel to patients' homes within service area.
