You will evaluate and treat acute and subacute conditions, perform transitional care visits including hospital discharge follow-ups, and coordinate care with primary care teams to improve access and outcomes.

Qualifications

• Current Massachusetts licensure as a Nurse Practitioner or Physician Assistant
• Board certification required
• Experience in primary care, urgent care or transitional care preferred
• Strong clinical decision-making and communication skills
• Ability to work efficiently in a dynamic same-day access setting

Responsibilities

• Provide same-day clinical care for patients in a fast-paced clinic setting
• Focus on transitional care visits, including hospital discharge follow-ups
• Evaluate, diagnose and treat acute and subacute conditions
• Coordinate care with primary care providers and care teams
• Patients scheduled continuously throughout the day
• Work 8-9 hour shifts with flexible scheduling options where available
