Job description

Responsibilities:

  1. Prioritize start of care visits
  2. Supervise Physical Therapy Assistants
  3. Next day mandatory reporting of start of care visits
  4. Clinical notes and other documentation due within 48 hours of visit
  5. Maintain timely communication with supervisor, intake, and office
  6. Be generally responsive during normal business hours

Requirements:

CA PT License and one year of Home Health experience

Territory: Sacramento, CA (Will be required to expand as needed)

Schedule: 40 hours per week, Monday-Friday and one weekend a month rotation

Training: 3 full days, one in house and two in the field

Documentation System: Kinnser

Mileage Reimbursement: mile (from first to last patient)

Parking and Toll Reimbursement: Paid

Company Car: Option with a decreased bill rate

Bill Rate: OT Rate: +

Paid after 30 points per week

Holiday Rate: +

On-Call Rate: N / A

Call Back Rate: N / A

Productivity Requirement: 30 points per week

Point to Hourly Conversion: 1 = 1.33hrs

Caseloads and visit mixes per day will equal to an average of 8 hrs per day and 40 hrs per week. Overtime must be pre-approved by the Clinical Supervisor.
