North Country Healthcare in Whitefield, NH is seeking a Primary Care Physician (Family Medicine or Internal Medicine) to join Weeks Medical Center as core faculty, combining outpatient care with resident education.

Qualifications

• Graduate from an accredited medical school with an MD or DO degree.
• Successful completion of an ACGME residency program with board certification in Family Medicine or Internal Medicine.
• NH medical license or ability to obtain license, BLS from the American Heart Association.

Responsibilities

• Provide comprehensive primary care, including exams, diagnosis, treatment, and care planning.
• Manage acute and chronic conditions with follow-up and monitoring.
• Deliver preventive services such as immunizations and wellness visits.
• Order and interpret diagnostic testing and adjust care plans.
• Prescribe medications and therapies per evidence-based guidelines.
• Educate patients and families on disease prevention and healthy lifestyles.
• Coordinate care with specialists and other providers for seamless care.
• Collaborate with nursing staff to ensure continuity and quality.
• Maintain timely documentation per Medical Staff Bylaws and policies.
• Participate in quality improvement initiatives and clinic call rotation.
