Job summary

Access Medical Clinic seeks a capable Advanced Practice Provider (APRN/PA) to serve as primary care provider in a Rural Health Clinic. The APP delivers patient-centered care across all ages, including pediatric and seniors, and promotes clinic services within the community.

The role involves comprehensive assessments, chronic disease management, treatment planning, and coordination with physicians and staff to ensure quality care and regulatory compliance in a fast-paced rural setting.

Qualifications

• MSN, DNP, MPAS, DMSc or equivalent advanced clinical degree.
• Active, unencumbered state license; DEA and BLS certification.
• DOT Certification.
• Ability to manage care across ages from pediatric to geriatric.

Responsibilities

• Perform comprehensive patient assessments, including history, physical exams, and diagnostic evaluations.
• Diagnose and manage acute and chronic medical conditions.
• Develop, implement, and monitor individualized treatment plans.
• Prescribe medications and therapies in accordance with state and federal regulations.
• Order, interpret, and review laboratory tests, imaging, and other diagnostic studies.
• Provide preventive care, health education, and patient counseling.
• Document patient encounters accurately and timely in the electronic medical record (EMR).
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate care.
• Refer patients to specialists and follow up on consults as appropriate.
• Ensure compliance with company policies, clinical protocols, and regulatory requirements.
• Participate in quality improvement initiatives and continuing education.
• Lead clinic staff in behaviors, actions and attitude in delivering excellent patient care.
