The CDC Foundation is seeking a Rural Health Transformation (RHT) Content Coordinator to support the Kentucky Rural Health Transformation Program (RHT). The Kentucky RHT is a multi-year, statewide initiative to modernize rural health systems and improve access, quality and sustainability for rural Kentuckians. The program operates across five interdependent initiatives - Rural Community Hubs, PoWER (perinatal health), Rapid Response to Recovery (behavioral health and crisis care), Rooted In Health (integrated/oral health), and Crisis To Care (community trauma care and EMS) - coordinated by a central program office.
This role combines hands-on content production with active participation in the communications strategies and execution plans that guide the program’s work, ensuring clear, consistent and coordinated messaging across a large, multi-vertical portfolio.
Working in close partnership with the Communications Lead, the Coordinator also serves as day-to-day coordination lead for the Engagement and Outreach Coordinator team, ensuring field-facing staff have the materials, messaging direction, and support they need to represent the program effectively in communities across Kentucky.
This project is 100% funded by the Centers for Medicare and Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) for a total of $212.9 million in budget year 1 with 100% by CMS/HHS.
