The RHTP Navigator will serve as a frontline, community-based member of a Local Community Hub within Missouri’s ToRCH Care model, coordinating clinical, behavioral, and social services while conducting screenings and managing referrals.

Reporting to the Hub Program Manager, you will document progress in CIE, support care plan adherence, and travel across homes, clinics, and partner sites within the assigned Hub geography.

Qualifications

• High school diploma or equivalent required; experience in community health, care coordination, or patient navigation preferred.
• Strong verbal and written communication skills; ability to document and track activities using digital systems.

Responsabilités

• Conduct standardized screening and intake to identify clinical, behavioral, and social needs; document findings, referrals, and follow-up in CIE and other systems.
• Manage closed-loop referrals and warm handoffs across Hub partners; confirm service receipt and re-engage residents if needed.
• Engage Medicaid members and high-need residents through calls, field visits, and partner-site follow-up; provide health education and care navigation.
• Help residents access Hub-supported services such as primary care, behavioral health, EPSDT, and telehealth.
• Identify and resolve barriers to care, including transportation, scheduling, benefits, and digital access; escalate when needed.
• Own assigned caseload and maintain structured follow-up until services are completed or appropriately transitioned.
• Contribute to Hub huddles, case reviews, and partner meetings; support provider coordination and referral pathways.
• Track outreach, screening, referrals, and outcomes data; share insights with partners.
• Escalate urgent concerns to licensed staff according to Hub protocols.
• Implement re-engagement strategies for hard-to-reach residents.
• Maintain a visible community presence to build awareness of Hub services.
