Mercy is seeking a Rural Health Transformation Program Navigator to serve as a frontline liaison helping residents access healthcare and social services in Dexter, MO. • Associate degree required; Bachelor’s degree preferred in Social Work, Public Health, Community Health, Human Services, or related field.
• Two years of experience in community outreach, case management, patient navigation, or social service delivery preferred.
• Strong interpersonal, communication, and problem-solving skills.
• Ability to work independently in community settings with limited supervision.
