- Completion of ACGME-accredited residency in Family Medicine, Internal Medicine, or Med/Peds.
- Board eligible or board certified in Family Medicine, Internal Medicine, or Internal Medicine & Pediatrics
- Licensed as MD or DO in NC or eligible to obtain license
