Community Solutions in Santa Maria, CA is seeking a Residential Re-Entry Housing Placement Coordinator & CBT Facilitator to identify housing options, create placement plans, and connect participants with community services. The role emphasizes motivational interviewing, life skills programming, and collaboration with agencies to help clients achieve stable housing and successful re-entry.

This full-time position offers benefits and an hourly wage of $26.45, with travel up to 25% and a schedule

Qualifications

• Must possess an associate’s degree from an accredited institution.
• Minimum of two (2) years of experience as a RRH Placement Coordinator or similar position; or four (4) years of experience as a RRH Placement Coordinator or similar position.
• Excellent oral, written and interpersonal skills desired.
• Experience with Microsoft Office.

Responsibilities

• Identify, organize, maintain, and disseminate housing information conducive to long-term recovery and stable housing.
• Locate market-rate housing opportunities from a variety of sources.
• Create individualized housing and placement plans based on participant goals and needs.
• Provide referrals to community housing and related support services.
• Follow up on housing applications and support participants throughout the housing search process.
• Facilitate housing meetings and review opportunities with participants.
• Ensure participants adhere to all RRH requirements.
• Facilitate life skills programming and motivational interviewing sessions.
• Evaluate participant progress and document outcomes.
• Collaborate with participants to develop and implement a Case Management Plan.
